Chaz’s Adult Circumcision

Below is the account I wrote the day after surgery, describing the experience from start to finish.  My procedure far exceeded an “average circumcision”, including a "scrotal recession phalloplasty."  Without a complete understanding of my specific special needs, you might be shocked, but please don't be!!  Do not let my photos frighten you!  If you want to skip the scrotal recession phalloplasty portion and go directly to the actual "sleeve resection" (circumcision) portion of the procedure, start here and keep clicking on "Next Picture" -- the only thing you will not see are the pre-op exam, administration of anaesthesia, frenulectomy (removal of the frenulum) and the scrotal recession phalloplasty (removal of the extra scrotal tissue.)  Everything went well, the circumcision itself had the fewest issues, the scrotal Z-plasty (scrotal recession phalloplasty) took the longest to heal.

 


Reflections on My Circumcision and Related Procedures

The night before surgery, I could barely sleep. Excitement and anxiety kept me awake as I counted down the last few hours. Leading up to the day, I had spoken with Dr Dave several times by phone and email to go over expectations and surgical goals. I had also received approval for Joe S to attend and take still photos. (Unfortunately, video wasn’t permitted; too bad, because watching Dr Dave work was astonishing.)


Arrival and Consultation

My appointment was at 1:00 PM. My stomach was in knots on the drive over, and I honestly thought I might vomit, but the anticipation—and the fact that I had dreamed about this for years—overpowered any second-guessing.

As soon as we entered the waiting room, we saw Dr David Cornell and Bart behind the sliding glass window.  They greeted us promptly and took us back to complete paperwork and begin the consultation. Dr David was exactly as I expected: professional, warm, and reassuring.

After a short discussion, I undressed for the exam. Although Dr David and I had already thoroughly discussed my goals, I restated them clearly:

A)     A prior injury had torn out my Prince Albert piercing, leaving a large (00ga, 9.2 mm), jagged hole through the frenulum. It had been poorly repaired by another surgeon. I hoped Dr David could close it cleanly, though he cautioned the size might make it difficult.

 

B)     My frenulum was elevated, tight, and frequently irritated. I wanted it removed.

 

C)    I had significant scrotal tissue extending up the underside of my penis, forming a penoscrotal web that caused chafing and tension—especially during erections. It also fused into the frenulum area.

 

D)    I wanted a high, tight sleeve resection circumcision, retaining as much mucosal tissue as possible while achieving a clean, even, circular line with a flat, unobtrusive scar.

Preparation for Surgery

Dr David reviewed the plan, looking a bit overwhelmed but fully confident. I signed the consent form, then Joe and I took a short break while the OR was prepped.

When we returned, it was time for the part I dreaded most: the anaesthetic injections. The initial shot into the pelvic area stung, but after that I barely felt the additional injections around the base and into the frenulum. I changed into a paper robe (thanks to George’s advice to leave socks and a shirt on, it was a lifesaver as the room is chilly).


In the Operating Theatre

The tray of instruments was impressive: clamps, scalpels, sutures, cautery tools. Eric applied a grounding pad to my right flank for the electrocautery device. Joe positioned himself to take photos.

Dr David tested the anaesthetic—only one tiny spot still had sensation, and a bit more numbing quickly solved that.

I lay back and without me even realizing it, the frenulectomy was already done—clamped and cut in seconds.

Next came closure of the former Prince Albert site. I smelled the cautery tool (like burnt chicken wings), but I felt nothing.


The Scrotal Web Removal

This was the most complex portion. Using the cautery device, Dr David removed a diamond-shaped section of tissue about 5 cm long.  Once it was gone, I felt my testicles drop lower onto my legs in a strange but unmistakable sensation. Because the web involved two layers of tissue and required recreating the natural raphe line with specialized stitching, this section took 45 minutes to an hour.


The Circumcision (Sleeve Resection)

Finally came Dr David Cornell’s signature work – the adult circumcision.

He began by making a precise circular incision, cauterizing small bleeders as he went. There were more internal adhesions than expected, and he addressed them carefully. He then cut along the line he had previously made, freeing the foreskin except for a small attachment on the underside. Once lifted and clamped, he removed it completely.

Watching the foreskin come off—knowing that was it, gone forever—was surreal and exhilarating.

He inspected the area, cauterized vessels, and then meticulously aligned the remaining skin. Clamps held everything perfectly in place as he used four sutures in key points and then applied steri-strips. After cleaning the area and applying a topical anaesthetic, he wrapped the penis in gauze and Coban bandage, which would stay on for 15 days.


Afterwards

When I stood up at 15:30, I urgently needed to pee and clean up a small amount of blood on my legs.  I realized—for the first time—I had a perfectly straight stream without needing to retract anything.  Yes, a small thing, but surprisingly exciting for me!

With the bandages in place, the next 15 days felt like waiting for Christmas morning as a child: I knew the reveal on January 25 wouldn’t be the final healed result, but it would already look far better than it had before surgery.

Around day 8, I changed the compression bandage and got my first small glimpse at my newly remodelled penis.  It was looking slightly worn but holding up, though it feels colder without the turtleneck.
