Most stories are from the patients perspective, but there is always 2 sides to every tale. I”m Doctor Gomco, and while i work in family practice my specialty is circumcisions I have a great conversion rate of uncut males walking into my room coming out cut

here is just one of my patients.

Josh is a teenage boy and keen rugby player. He has been lucky to be picked for his local teams starting side, but before hand needs a medical clearance. He is booked to my clinic as the last patient on a Friday afternoon.

Josh strides into the room in his school uniform, shirt untucked. Just over 6 foot tall and blond, with the muscular build of someone that spends more time on the sporting field and in the gym than paying attention at school. His father attends with him today, and while not as tall or muscular there is still a family resemblance.

Josh handsover the form and i look at it briefly. I’ve seem similar before, and while it contains all the normal ‘chest’ ‘limbs’ ‘head’ boxes, it also thankfully contains one saying ‘genitals’.

I go through the normal history with josh, and his is very healthy - just some mild backache at times. as i shift to the urinary system the questions become more personal. ‘pain when you pee’ ‘no’ ‘blood when you pee?’ ‘nope’ ‘pain when you ejactulate?’ ‘ah…no’ he replies with a smirk ‘are you circumcised or uncircumcised?’ ‘ah….uncut’ he replies quitely. dad nods in the background. ‘any issues retracting your foreskin?’ ‘no, not at all’ also quiet. i move back to less explicit ground with a few questions about diet and bowel habits. when that is over it’s time for the exam. i check his head and neck, and then josh removes his shirt while i listen to his heart and lungs. i check glands and reflexes with his upper limb. he has a light covering of blond hair on his chest. i request that he remove his pants but leave his underwear on. he does so and i check more reflexes and glands i ask that he gets up on the bed, and i feel his abdomen, which is muscular but normal. finally i request that he lower his underwear josh does so slowly, allowing his penis to flop out. he has trimmed his pubes down fairly short, but there is still a small halo of blond hair. his balls however are shaved. his penis is somewhat above average at rest for length but average girth, and is certainly uncut with the foreskin extending just slightly beyond the head. i examine his testicals for lumps for which there is none. ‘josh can you retract your foreskin for me?’ he does so gingerly, and it slides back without any particular trouble ‘hmm i say - may i try?’ he does not say anything but nods a tiny amount and lets his foreskin return to the natural position i retract for him ‘it feels tight to me’ i say ‘um, not really’ ‘but when you’re erect….?’ he blushes then mutters ‘ah…maybe, i don’t know’ “is there a problem” josh’s father asks? ‘josh has a tight foreskin’ ‘oh i see’ dad frowns josh you can pull up your underwear now, and get dressed if you like ‘is it really a problem?’ dad asks ‘yes’ i reply simply. ‘and will become more in future’ ‘i never noticed’ dad mutters ‘can be hard to tell when boys are younger’ ‘i always told him to clean it, i guess i thought that was enough’ ‘are you circumcised Mr Ryans?” ‘ah yes’ ‘would be even harder for you to know then. is there a reason Josh was not circumcised like you?’ ‘oh my wife….she wasn’t so keen’ ‘well i’m afraid there is little choice now’ ‘what!” josh says suddenly paying very close attention ‘it’s a medical problem, you will need it sorted before i can give you clearance to play rugby’ ‘but i don’t want to!’ ‘now now josh’ dad says trying to calm things down. ‘the doctor is just trying to help you’ ‘of course’ i say josh slumps dejected ‘is there any other option?’ dad enquires ‘i’m afraid not, but it’s a quick procedure. short recovery’ ‘i see. well we better get it sorted then Josh?’ Josh’s only reply is a grunt ‘the sooner the better really, so you can get back to your sport’ i say as i’ve learnt, it’s much better not to give them much chance to reconsider ‘in fact i can do it for you now, you would have the weekend to recover?’ Josh goes completely pale ‘well that would actually be fairly convent, Josh? what do you say?’ ‘i….i was going to do stuff’ he says eventually ‘but good to get it out of the way, yeah? dad says. ‘ i think you should do it now josh’ ‘so josh, what it would involve is a procedure to take off the skin that is too tight - it’s diseased and it all needs to go. you would end up pretty similar to your father i would imagine.’ ‘its probably cleaner josh’ dad says ‘and like the doc said you need it’ ‘i can give you men a minute while i setup in the procedure room, and if you dont’ want to go ahead thats fine’ i step out of the room and hear some gentle chatting. i give them 5 minutes as i setup my clamp and sterile gear. when i step back in joshes eyes are a little red, and he looks stressed but he agrees to go ahead

we go through to the room and i request josh strip again i tell dad he can sit at the head of the bed while i do it i setup a small curtain so josh cannot easily see what is happening ‘this will be cold, but i’m not doing angythibg yet’ i prepare the area with antibacterial wash ‘the local anaesthetic stings, but it will soon fade’ i start injecting and Josh clearly feels the first prick and few mL, but then his body does not tense anymore as i continue ‘easy’ i say ‘while i wait for it to work, i’ll explain what i’m going to do - so i use a special clamp to help me do it well. its very important not to leave any scared skin behind otherwise you could have problems in future, so you will be getting a VERY tight cut. it will be a change, but you’ll grow used to it. also as standard, i take out the frenulum, it does not do anything anyway. ‘i’m going to start now’ i grab a force and clip his foreksin roughly. given that he does not move, i know that the local has worked well. i make a slightly snip with some scissors on the dorsal aspect to more easily manoeuvre the clamp, and then i grab my trusty gomco and slid it over his glans ‘a bit cold buddy’ then the foreskin is pulled back very firmly until his shaft skin is tense and then i pull it an extra centimetre for that ultralight cirumcision the bell is applied and pressure is exerted

his foreskin is being squeezed to death

it only take a minute and the damage is done,

finally i use a knife to cut around the line and his foreskin is disconnected

pulling the foreskin away, only the frenulum is left and with scissors i cut that down to the shaft, completely eliminating it.

the foreskin goes in a jar, and i begin sowing up the edges.

because of the position of the clamp Josh has much inner mucosa left, but little shaft skin. he has been given a high and tight cut. i begin doing subcuticular stitches and as the inner skin is joined to the outer his glans already flares significantly

i clean the area of all the blood

‘ok josh, time for you to have a look - and then i’ll put the bandage on’

i lower the screen and Josh discovers what his penis will look like forever more

‘fuck’ he says

“josh!’ dad replies

his penis has been skinned and he knows it, i ignore the profanity

i wrap it up gently

‘so it will heal up ok?’ dad asks

‘oh yes, come back in 2 weeks’ and i wrap it up.

(2 weeks later)

Josh and his father again walk through the door

after some pleasantries, i ask how its all going

josh has been very quiet

‘it doesn't feel the same’ he pouts

may i see?

josh drops his pants and i see my work on display: a perfect high and tight, with not a millimetre of loose skin, the frenulum expunged.

‘its not as senstive’ he says

i smile ‘that will come back in time’ fully aware that it won’t. he’s stuck like that. ‘i suggest you use lubricant if you do not already’ i say josh appears confused, but dad clearly knows what’s up - ‘i’ll talk to him later about that then’ he replies. ‘i’ll just sign this form then. and best of luck with the season’ i say to Josh as they both leave.
